High-Risk Pregnancy Care

Bringing Structure, Awareness, and Data to High-Risk Pregnancy Care

Maternal health programs around the world face a familiar challenge: screening tools can detect early risk, but the information they generate rarely connects to patient support, clinical decision-making, or population insight. This is especially critical in conditions like Preeclampsia, where early detection only makes a difference if risk is monitored, acted upon, and understood over time.

Intignus Biotech recognised this gap and envisioned a connected maternal health infrastructure - one that collects and analyses women’s health and screening data through a unified platform, designed to improve patient care, strengthen clinical decision-making, and enable public health insight

This infrastructure unifies risk screening, ongoing patient engagement, and standardised data that can support clinicians and inform public health priorities. Their goal was not to build a standalone app, but a pathway capable of scaling across hospitals, supporting clinicians with future diagnostic tools, and strengthening population-level understanding of maternal risk.

To bring this vision to life, Intignus partnered with Avegen as a technology collaborator. Using the HealthMachine™ platform, Avegen built a secure, clinically guided digital foundation that could evolve in step with the needs of patients, clinicians, and health systems. The result is WiseMother: a digital pathway designed to support care, generate meaningful insight, and scale with the future of maternal diagnostics.

The problem

When maternal risk data is fragmented across paper files, sporadic appointments, and unstructured communication, early detection loses its impact. If women are screened but not supported, if clinicians cannot see how risk evolves between visits, and if public health systems cannot learn from real-world cases, the opportunity to prevent avoidable complications is lost.

WiseMother helps bridge this gap. It transforms a single risk-identification moment into an ongoing pathway that supports:

  • Women through awareness, guidance, and structured follow-up.
  • Clinicians through standardised assessments and evolving patient context.
  • Public health systems through consistent, anonymised data that can strengthen maternal health planning.

The aim is not only safer pregnancy monitoring today, but a foundation for stronger maternal health systems tomorrow. This includes generating standardised data that, over time, can support public health insights and inform maternal health program planning.

The Challenge

Addressing high-risk pregnancy requires more than detecting early signs. It demands a system that supports women, equips clinicians, and generates data that can strengthen maternal health programs. WiseMother was designed to respond to gaps across all three levels.

Clinical: Early warning signs of hypertensive complications can progress quickly, yet the information clinicians rely on is often scattered across paper files, memory, or sporadic visit notes.

Without a continuous view, important patterns may be missed.

Behavioural: Many women, especially first-time mothers, may not recognise symptoms early enough or may underestimate the importance of attending scheduled follow-up visits.

This leads to missed follow-ups and unmanaged risk - especially when clinical visits are infrequent.

Operational: Intignus needed a way to support a structured pregnancy pathway: registering women seamlessly, prompting timely visits, and collecting consistent, anonymised data that could improve both immediate care and future diagnostics.

Strategic: The partnership aimed to create a program that united education, symptom tracking, and follow-up adherence - all within a trusted digital environment.

The goal was not only to digitise the existing workflow, but to create a scalable, data-driven programme that could support patient care, assist clinicians, and ultimately inform public health priorities.

Our solution

WiseMother is built to reflect Intignus’ vision for a unified maternal-health data and care pathway, and the HealthMachine™ platform enabled a swift, compliant, evidence-guided build that supported their clinical and public health objectives.

The result is a scalable digital foundation capable of supporting Intignus’ evolving diagnostic tools and expansion across multiple hospitals.

WiseMother is already engaging with clinicians and healthcare workers who are providing direct inputs for the platform’s development, ensuring that its features and workflow align with real-world practice.

For pregnant women

The WiseMother mobile app acts as a companion throughout pregnancy.

Intignus has developed educational content with the aim of improving health literacy in women that was disseminated through the app. Women can learn about hypertension-related risks, understand what symptoms to watch for, and track daily wellbeing using structured, clinically guided inputs. The app also supports timely reminders for the recommended pregnancy check-ins aligned to key pregnancy monitoring stages. These appointments typically take place at different points across pregnancy - early, mid, late, and near-term, and are designed to monitor changes in blood pressure, symptoms, and other indicators linked to hypertensive complications. By helping women remember and prepare for these visits, the app supports stronger continuity of care and reduces the likelihood of missed assessments during critical periods of maternal risk.

A simple dashboard connects everything together, allowing women to understand how their wellbeing changes over time.

For Clinicians

The clinicians use a secure web interface to register women and complete structured digital assessments during each clinical visit.

The standardised touchpoints help create a continuous record of high-risk indicators, offering clearer visibility into how symptoms change over time. Meanwhile, anonymised aggregated data offers early insights into user behaviour and pathway engagement, helping identify whether those with elevated risk of Preeclampsia remain connected to the recommended monitoring and supported journey.

A connected clinical workflow

The digitised care pathway ensures that key health indicators are captured consistently at every stage of pregnancy. Each visit contributes to a continuous health record that shows key indicators evolve over time, something that a one-off screening cannot provide.

Subtle changes in symptoms or wellbeing can offer early clues that guide timely advice or escalation, supporting clinicians before complications become harder to manage.

In Intignus’ Own Words

"Avegen translated our vision into a working solution, delivering a prototype that validated our core assumptions quickly. Their pragmatic execution, clear documentation, and collaborative handover created a reliable process that de-risks scaling from pilot to enterprise and opens up opportunities for expanding to new use cases and geographies. This partnership accelerated our entry into digital health and gave us a solid, scalable foundation to grow confidently."

Results

Today, WiseMother is established at four partner hospitals; a foundational milestone that supports future expansion without positioning current scale as the end goal.

Adoption and Reach

In its early rollout phase, WiseMother has:

  • Onboarded 94 users
  • Supported enrolment at 4 clinical sites
  • Established a foundation for broader multi-site deployment.

Key Success Metrics

For Intignus, WiseMother’s success is defined by its ability to support expansion across hospitals and clinical teams, rather than patient usage numbers alone. Early engagement data offers a baseline to refine the pathway and strengthen its readiness for broader clinical adoption:

  • The average session duration of around 3.5 minutes suggests that women are interacting meaningfully with content and tracking features.
  • 12.5% 28-day retention reflects a promising start for a newly launched maternal program.

These results represent the program’s foundational stage, where where usability, stability, and pathway adoption matter more than scale. The data provides early behavioural signals that will guide refinement, clinician-facing features, and expansion across more hospitals.

Key Takeaways

  • WiseMother supports continuity across clinical, behavioural, and public health needs
  • Real-world data helps Intignus refine the pathway and inform future diagnostic tools
  • The program is built for iterative improvement and scale across multiple hospitals and health systems.

WiseMother demonstrates how a thoughtfully designed digital pathway can strengthen continuity in high-risk pregnancy care by supporting women beyond the initial screening moment.

As results continue to shape future enhancements, WiseMother stands as a collaborative example of how digital tools can deliver meaningful support, clearer insights, and scalable potential within maternal health.
